Community Health Care Team

Our Community Health Care Team includes:

Health Visitors

Our Health Visitors are based in the surgery. Health Visitors provide health care advice and support to parents of children aged 10 days to 5 years. The Health Visitor can be contacted through the practice.

District Nurses

The District Nurses perform nursing duties at home for patients who are housebound. Their help is arranged by the doctor or by the hospital for newly discharged patients.

Community Midwives

Our Community Midwife cares for expectant mothers and visits mothers and babies for up to 28 days after delivery.
