Aged 16 and over? We need to hear from you about your confidential information
You may not be aware but when you turn 16 you become responsible for your own healthcare decisions. We are required by law to ensure that medical data be treated with great respect for confidentiality. Confidentiality means that your consultation and visits to the surgery will not be discussed with anyone apart from you. The only time that information may be shared without consent is when you could be considered a danger to yourself or others

It is very important that we hold up to date contact details for you. Please ensure that we have your correct telephone numbers (home and mobile) and have a personal email address for you. If you have been sharing a mobile phone number or email address with a parent or guardian, these details will need to be removed from your record.
If you are happy to keep sharing your information with a parent or guardian, or someone else you may choose, you can.
Please come to reception and ask for a ‘Young Persons Consent Form‘ which you will need to complete and sign. We can then update your medical records to reflect your wishes.
